If you would like to complete a short evaluationof any Bible Basic Lesson or an entire course, copy this document and paste it into the "Contact Us by Email" email text box on the link to the left. Then type in your answers to the five questions listed below. Email the completed evaluation form. There will be no identification on the message. If you prefer to let us know who you are, copy and paste this form into your own email text box and mail to drh@churchsmallgroupmanagement.org
After pasing this blank form into the email text box, just type your evaluation statements into the spaces for the five evaluation answers. There are no correct or expected responses. The staff member who oversaw your submissions will read the evaluation for improvement efforts. Remember, we are looking for ways to improve the course for students who follow.
